If you want to ask politely, you can say :
ワクチン打ちました?
wakuchin uchimashita

- vaccine = ワクチン wakuchin
- got the vaccine = ワクチンを打った wakuchin o utta
“打った utta” is the past tense of “打つ utsu”.
We do not usually say “あなたは anatawa (= you)” in everday conversation.
