I have a dentist appointment.

歯医者の予約があるんだ。

 haishano yoyakuga arunda

If you want to speak politely, you can say :

歯医者の予約があります。

 haishano yoyakuga arimasu

  • I have … = わたしは … がある watashiwa … ga aru
  • a dentist appointment = 歯医者の予約 haishano yoyaku

We do not usually say “わたしは watashiwa (= I)” in everyday conversation.
